You can get the card easily from the branch where you opened your account. Just go to the branch, fill up the Application Form and submit it with required documents and a recent photograph. Eligibility
- TIN is mandatory.
- Age: Basic card - 21 to 70 years, Supplementary card - 18 to 70 years.
- Minimum Income: Salaried Group - BDT 15,000.00, Self Employed Group - BDT 20,000.00 (income should be validated).
- Business/ Service Tenure: Salaried Group - Minimum 6 months, Self Employed Group - Minimum 1 year.
- Bangladeshi Passport: All Group - For dollar endorsement to the value for which credit card is issued under Travel Quota entitlement.
- Declaration: All Group - A declaration from the intending card holder stating that he is not availing himself of any International Credit/ Debit/ Pre-paid Card from any other AD against the same account or the same entitlement.

Salaried Group
- Two copies Color Photograph (P.P. Size) duly attested by the applicant.
- Salary Certificate.
- Last Six Months Salary Account Statement.
- TIN Certificate.
- Copy of National ID card.
- Copy of driving license (If any).
- CIB Undertaking Form.
- Photocopy of Passport (1st to 7th pages & dollar endorsement pages).
- Proof of Residence (Electricity/WASA/ Gas Bill, if any).
- Proof of Car ownership (Photocopy of Blue Book of the car, if any).
- Proof of T&T ownership (Photocopy of T&T Bill, if any).
Self-employed Group
- Two copies Color Photograph (P.P. Size) duly attested by the applicant.
- Valid Trade License (if Proprietor)/ Partnership Deed (if partnership)/ Memorandum of Articles with Incorporation Certificate (for Ltd. Co.).
- Current six months Bank Statement (personal & Company).
- TIN Certificate (only personal TIN).
- Copy of National ID card.
- Copy of driving license (If any).
- CIB Undertaking Form.
- Photocopy of Passport (1st to 7th pages & dollar endorsement pages).
- Proof of Residence (Electricity/WASA/Gas Bill, if any).

Card Activation
- After receiving the card, sign the acknowledgement slip and send it to your home branch for activation.
- Put your signature with a ball point pen on the signature panel at the back of your card.

Caution during making transactions
- Please do not let your card be taken out of your sight at any merchant outlet to prevent possible Misuse.
- After using your card at a merchant outlet, please ensure that the card returned to you is yours.
- Before signing the Sales Slip, check the amount charged.
- Retain your copy of the Sales Slip of all transactions until they appear in your Card Account Statement for your future reference.
PIN Maintenance
- Destroy the PIN mailer after memorizing your PIN.
- Do not write the PIN on the card or keep the PIN inside your wallet.
- Change the PIN preferably every month.
For Lost/ Stolen Card
To report and replacement of your lost/stolen Debit Card, please contact your nearest Branch. Your card will be replaced within 7 working days and will be sent to your Branch.
